How long does a worker have to give NOTICE of their injury to their employer?

NOTICE–the second crucial element of ANCR and what you need to file your workers' compensation claim.  You MUST notify your employer about your accident. How long do you have until your claim is invalidated? In what form must this notification occur? What about with injuries or conditions that develop later after the injury?
