Former teacher with back, neck and shoulder injuries.

Claimant alleged disability beginning in January of 2010 due to serious pain symptoms in her back, neck and right shoulder. In May of 2009, the claimant suffered a serious car accident where she was injured. Although she continued to work after the accident, her injuries eventually forced her to stop by January of 2010. Claimant worked as a 2nd grade school teacher for the last 22 years. As a result of her back and neck injuries, claimant received treatment from an orthopedist, pain management physician and physical therapist. She received epidural steroid injections, physical therapy and monthly evaluations from her doctors.
