How does an appeal work in workers’ compensation?

What if you, or the insurance company, doesn't like the decision given in the initial hearing? Both parties have the right to APPEAL  a decision in hopes that they will get a favorable decision.  Who is this appeal made to? How long does it take? More importantly, what happens to the injured worker as they are waiting for a decision? What happens if you return to work during this time?
