It’s important to note there are two types of applications for retirement disability benefits under the New York State system: Article 14 ordinary disability benefits and Article 15 disability benefits, which are generally for employees who have been on medical leave or who have been receiving workers’ compensation. You must file an Article 15 Disability Retirement Application (RS6340) within 90 days of receiving a paycheck or 12 months after termination.
